Registration Forms

I. Eligibility

To be eligible to play for the City of Angels Little League, children must have residence within CALL’s boundries (South of Hollywood Boulevard; North of the 10 Freeway, between Vermont and Crenshaw; North of Venice, between Crenshaw and LaBrea; East of La Brea Avenue; and West of Vermont) and must be between the ages of 4 and 14, as of April 30, 2010.

Each child must provide the following:

1. Proof of Residence – 3 items from the below list

School Records
Utility Bill (Water, Electric, Gas, Phone)
Cable Bill
Cell Phone Bill
Driver’s License

2. Certified Copy of Birth Certificate (blue copy) or Passport

II. Registration Procedure

There are three (3) ways to register: in person, via mail or on-line.

To register in person:

Please fill out the player registration and medical release forms (download from menu to the right). When completed, please bring both forms, along with a check for the Registration Fee, made payable to "City of Angels Little League" ($60.00 before March 1, 2010; $75.00 from March 1, 2010 until the end of the 2010 season) to:

To register via mail:

Please fill out the player registration form (download from menu to the right). When completed, please send the application, along with a check for the Registration Fee, made payable to "City of Angels Little League" ($60.00 before February 28, 2010; $75.00 from March 1, 2010 until the end of the 2010 season) to:
